Investigation Summary
On August 4, 2006, Employee #1 was operating an in-line Kayat Packaging Machine, Model Number TP-50A, Serial Number 176-98. The Kayat stopped and started automatically, depending on supply and demand required for placing six bottles into a cardboard container. Employee #1 observed that a spatula had fallen off the conveyor and landed in the machine's undercarriage. When the Kayat stopped momentarily, Employee # 1 reached blindly into the Kayat undercarriage to remove the spatula. The Kayat started automatically, and his right hand became caught in a chain and sprocket, partially amputating his right thumb. He also had unspecified fractures and lacerations to his hand and fingers.
