Accident Investigation Summary
Summary Nr: 201149200Event: 11/02/2006Mechanic'S Legs Are Pinned Under Tractor-Trailer Wheels
On November 2, 2006, several employees were transporting a 350-ton turbine utilizing a 200-ft long trailer (transporter) being driven by two Mack tractors on eastbound Highway 91 near the Lakeview Ave intersection. The company was engaged in the rigging and transportation of heavy equipment. Due to equipment malfunction, the crew stopped the transport of the turbine, and found that one of the airlines controlling the brake system on the transporter was damaged. After the crew performed the necessary repairs to the damaged airline, the crew mechanic situated himself next to a tandem set of trailer wheels to reconnect the quick coupling of the airline. The residual pressure in the airline released the brakes on the set of wheels causing its inadvertent movement. The mechanic's legs were pinned and crushed under the wheels. He was hospitalized for 32 days.
