Grinder Wheel Shatters and Lacerates Employee's Leg

At approximately 9:20 a.m. on June 28, 2008, Employee #1, returned from a break and started his grinder. Employee #1 applied the vertical portable grinder to a draft sill casting (Part Number S4779772), and the 9-in. grinding wheel blew apart. A piece of the wheel struck Employee #1 in the right thigh. Employee #1 sustained a deep laceration to the right thigh requiring hospitalization and surgery.
