Employee Is Killed

At approximately 11:43 a.m. on December 13, 2006, Employee #1 was working at a sawmill. An approximately 7-ft long by 24in. diameter red oak log was placed on a log deck conveyor for processing. The log deck conveyor consisted of three concurrent 2-in. conveyor chain lines actuated by a single hydraulically driven axle. The control to actuate the log deck conveyor consisted of a foot control located inside a cab which also operated the main saw and edger saw controls along with ancillary functions. The cab was operated by the sawmill owner. Weather conditions: 53 degrees Fahrenheit; 63-percent humidity; 9 mile-per-hour winds. The log became positioned askew on the log deck conveyor as it was being moved towards the saw. Upon conferring with the Owner and another employee, Employee #1 rigged a chain around the askew log while the coworker went to secure a powered industrial vehicle in an effort to clear the log by pulling it off of the log deck conveyor. As Employee #1 had either finished or was still rigging the chain around the log, it either rolled or slid from the conveyor deck, striking Employee #1. Employee #1 sustained massive skull and facial fractures due to blunt crushing injury. Employee #1 was killed.
