Investigation Summary

Investigation Nr: 200841260
Event: 09/11/2006
Employee Is Killed When Struck by Semi-Truck

At approximately 12:07 p.m. on September 9, 2006, Employee #1, an independent truck driver, was working in the dock area at the Georgia-Pacific company. The dock area was handled by another company, the National Freight Incorporated. Another driver, who worked for National Freight Incorporated, backed into the stall area, which was crooked. He saw Employee #1 sitting in his truck. Employee #1's truck was adjacent to another driver's semi-truck's trailer. They made eye contact. Another driver pulled his truck back out to straighten the trailer. When he backed again into the stall, he was yelled out that Employee #1 was pinned by the trailer against the loading dock. Employee #1 was killed. No one witnessed the accident. The truck driver did not see Employee #1 getting out of his truck. The location of the accident was the one open stall between Employee #1's trailer and another parked trailer.
