Investigation Summary
On September 29, 2005, Employee #1 was on the roof of a house, installing the drip edge on the rake edge of a dormer. At some time after lunch, he fell approximately 8 ft from the dormer roof to the garage roof, and then fell another 11 ft 5 in. from the garage roof to the ground. He sustained injuries that required hospitalization. At the time of the accident, Employee #1 was wearing a rock climber's safety belt along with a rope grab. He later stated that he had slipped while working, but that the rope slid through and out of the rope grab, allowing him to fall to the ground.
