Investigation Summary
Sometime after 10:07 a.m. on August 15, 2005, Employee #1 was performing cycle counts in the inventory department of a firm. Finding a discrepancy, he had gone from a dock to a lot where trailers were located in search of a non-conforming part. This part would have been in either trailer Number 17 or trailer Number 20. Employee #1 was last seen at break time from 9:00 a.m. to 9:30 a.m. in the lunch room, and the company's surveillance camera showed Employee #1 leaving via the dock door at 10:07 a.m. Around 12:00 p.m. (noon), a FedEx driver pulled up and told a coworker of Employee #1's that someone was sleeping in the lot out back by a trailer. The coworker went to the trailer and found Employee #1 lying on his back with his arms folded across his chest. Employee #1 had blood all over his face, and there was blood on the ground around his head. Emergency medical services were called. Employee #1 was found behind trailer Number 20, with its right back door open and the lock and keys sitting on the bumper of the trailer. It is unknown if Employee #1 was inside trailer Number 20. When Employee #1 was found, he was lying with his head toward the right rear portion of trailer Number 20 and his feet pointing away from the trailer. His glasses were just under the rear bumper of the trailer, approximately 3 feet away. The preliminary autopsy report was that Employee #1 had a fractured skull, apparently from falling. The fracture was located in the center of Employee #1's head, which caused Employee #1 to bleed to death. There were no witnesses to the incident.
