Employee Is Killed When Struck by Load from Forklift

On August 24, 2005, a stack of light collapsible plastic totes, each weighing 208 lb and measuring 15-ft-high, was being transported by forklift. The load which was estimated to be 10 ft high became unstable and tipped over with one or more of the totes, striking the employee. The employee was killed.
