Investigation Summary

Investigation Nr: 201692662
Event: 03/26/2007
Employee Is Injured in Fall from Ladder, Later Dies

At approximately 9:40 a.m. on March 26, 2007, an employee was climbing up a ladder to the Electronics Room on Crane Number 43 to gather information to upgrade the crane's computer systems. He fell off the ladder approximately 20 feet to the ground. He suffered broken bones and internal injuries. He was transported to Harbor General Hospital. At Harbor General, the employee lapsed into a coma and never regained consciousness. He was transferred to Cedars Sinai Hospital, where he died of multiple organ failure on May 10, 2007. A coworker was with the employee and climbed the ladder before the employee did, but did not see him fall. The coworker reported the ladder's condition to be normal. No grease, water, etc that would have caused the accident. There were no other witnesses to the accident. The cause of the accident was not determined. The employee's supervisor was not on site.
