Investigation Summary
Employee #1 (the owner), and two other employees, were installing lintels over a bay widow at a residential home under construction. The lintels weighed approximately 250 pounds each. Employee #1 (the owner), and the employees, had set up a two-buck-high scaffold, using two planks at the 7-ft level resting on the built in ladder rungs. At the 10-ft level, planking was in place but being used as a work table to stock the lintels. The planking ends on this level were extending approximately 3 ft over the scaffold end. The employees were standing on the planks at the 7-ft level, attempting to set the lintels. They were having trouble handling the lintels themselves so Employee #1 attempted to climb up and assist them. Employee #1 climbed the built-in ladder and attempted to pull himself up to the 10-ft level from the planking that was extended over the end of the scaffold. The planks cantilevered causing Employee #1 to fall to the ground. When Employee #1 landed, a lintel that was stored on the plank fell onto his lower left leg, causing a fracture. Employee #1 was transported to a nearby hospital where he underwent surgery to repair the broken bones. Employee #1 was hospitalized for approximately one week.
