Investigation Summary
Employee #1, a semi truck-driver, was operating the hydraulic controls, between the front and back wheels of the trailer dolly he was using to transport a 60-ft-long tank. Employee #1 was trying to turn the heavy load around a corner manually. The back eight wheels of the dolly trailer rolled over his legs. Employee #1 sustained crushing injuries and was hospitalized.
