Investigation Summary

Investigation Nr: 201581055
Event: 03/19/2010
Employee gets fatally injured by industrial vehicle

At approximately 3:00 pm, on March 19, 2010, Employee #1, of Intermech Inc., was carrying an oxygen/acetylene hose and was walking uphill across a roadway. Employee #2, of Kiewit General, was driving a CAT 966E end loader downhill with a load of fill dirt. Near the bottom of the hill where the road curves, Employee #1 was almost across the road when the end loader struck him. Early reports indicate that apparently the bucket of the end loader struck him in the head and spun him around and under the wheels of the end loader. Employee #1 was fatally injured.
