Investigation Summary
At approximately 1:15 p.m. on October 4, 2005, Employee #1 was on an aluminum ladder installing a vinyl sign on a structure. He had tied off one side of the sign and moved the ladder to the other side to tie it. The ladder contacted an exposed electrical wire. Employee #1 was electrocuted and fell 15 ft to the concrete parking lot.
