Employee Is Killed When Caught by Logging Machine

At approximately 6:00 a.m. on August 25, 2005, an employee, a welder, was repairing the grabbing mechanism of a tree felling vehicle. The employee asked another coworker to close the arms of the grabbing mechanism. The coworker left the employee standing near the arms of the grabbing mechanism, but not between the arms, to walk around the side of the vehicle in order to access the cab. The coworker then reached into the cab and engaged the arms, then returned to the front of the vehicle to find the arms of the grabbing mechanism closed on the employee. The employee was crushed and died at the scene. The employee was a trained and experienced welder and logger with 5 years experience and had repaired logging equipment several times in the past. The employee positioned his person between the arms of the grabbing mechanism in order to conduct the weld repair. The employee repaired the equipment, while the vehicle was running and without the benefit of lockout-tagout procedures and protection.
