Investigation Summary

Investigation Nr: 200373504
Event: 06/30/2005
Employee Died from Unknown Reason

On June 30, 2005, Employee #1, a roofer, and a roofing crew were installing a hot tar roof on a newly constructed building. The morning was hot, sunny, and humid. After lunch the crew returned to work and approximately 30 minutes later Employee #1 told the superintendent that he was not feeling well and was going to lay down in a work truck. The superintendent and others checked on him for the rest of the work day and made sure that he drank much water. At approximately 5:00 p.m. the crew was completing the day and Employee #1 went to several other work trucks to cool them off. He got in the rear seat of one of the trucks and vomited. The Environmental Management System was alerted and as they were responding, Employee #1 became unconscious. After being transported to the hospital he died.
