Forklift Rolls-Over and Kills Operator

On May 9, 2006, Employee #1 was operating a forklift truck. Employee #1 was not trained nor had been required to demonstrate his capabilities prior to being allowed to operate a forklift truck. During operation of the forklift, Employee #1 was traveling with the forks fully extended, in an upward position. Employee #1 lost control of the vehicle, and the forklift tipped over. The forklift crushed and killed him.
