Employee Is Killed When Struck to Head by Rack

On April 4, 2005, Employee #1, a metal sawing machine operator, was standing just off of the aisle, on the way where a forklift was carrying an 860-lb metal die rack. The forklift passed behind her. The second metal rack was resting on the forks. Since the rack was not secured to the forks or mast, it became unstable. The rack rotated around the forks and struck Employee #1 in the back of the head. It pinned Employee #1 to the saw, face down. Employee #1 was killed.
