Investigation Summary
On March 10, 2005, Employee #1 was working in an excavation (8-ft long, 8.25-ft wide, and 7-ft deep) in Type C, water saturated soil. The trench's support system was not designed by a registered professional engineer. As two other employees removed the support system from the excavation, a section of the west wall caved in and slid under the raised support system. Employee #1's lower left leg was pinned against a water line. He was hospitalized for fractures of his lower leg and ankle, as well as lacerations.
