Investigation Summary
At approximately 10:00 a.m. on February 12, 2005, Employee #1 was installing exterior dense glass sheathing on the third floor of an apartment under construction. He was working from an unguarded two foot extension at the front of a work platform that was attached to a rough terrain forklift when six sheets of the glass sheathing tipped over from the back of the work platform. The sheets knocked him off the platform and he fell approximately 29 feet and was killed. He was not wearing a personal fall arrest system at the time of the accident.
