Employee Is Killed When He Falls from Aerial Lift Truck

On March 18, 2005, Employee #1 was using an aerial lift truck to install light bulbs on a covered basketball court. He was not wearing any fall protection equipment or other personal protective equipment. The circuits that control the lights were not locked out. Employee #1 fell from the platform and died.
