Investigation Summary
At approximately 10:01 a.m. on March 24, 2005, Employee #1 was inside a closed container removing a bundle of six granite slabs. As he was leaving the container, the fork lift operator started to move the bundle of granite slabs. The bundle of slabs shifted and trapped Employee #1 against the side of the container. Employee #1 died from the crushing injures to his chest from the granite panels that weighed approximately 6,000 lbs.
