Employee Collapses and Falls from Forklift and Dies

On July 23, 2007, Employee #1 was working from a stand-up forklift. He collapsed and fell from it. Employee #1 was killed. Initially, it was believed that he had heart attack. However, the autopsy revealed that he had blunt trauma to the back of the head. The accident area was monitored contaminants and heat stress. The results did not reveal any problems.
