Investigation Summary
Employee #1 was connecting a pipe from a large tank of yogurt to a main supply line so that it could be transferred to other tanks. The process involves removing a cap and making the connection. Employee #1 assumed a valve was closed when he removed the cap. The valve had not been closed and milk at a temperature of approximately 180 degrees Fahrenheit gushed out and burned him.
