Employee Is Killed When Steel Tube Bounces

On April 21, 2005, Employee #1 and other crew members were installing an 8-ft by 0.625-in. by 9-ft 3-in. steel tube, approximately 29 ft 9 in. above ground, between two tube columns. Each column had an "L" clip, where the tube steel was to set. The left side was secured on the "L" clip. While placing the right side on the "L" clip and down into the hole, the left side popped, shearing the stud. The tube steel went down, striking the ground. It bounced, striking Employee #1, who was on the ground. Employee #1 sustained a concussion to his head and was killed.
