Investigation Summary
On February 25, 2005, Employee #1 was working in an 18-ft-deep trench installing a "T" connector to connect a new 8-in. sewer pipe to an existing sewer. According to a coworker, Employee #1 stepped out of the trench box to reach for and install a piece of pipe needed to make the connection. While outside of the trench box the wall of the trench collapsed and covered Employee #1 resulting in his death.
