Employee electrocuted while installing light fixture

On February 22, 2006, Employee #1 was working at a building under construction in Macomb, IL. He was on an 8 ft ladder, installing a light fixture in the building's front office area when he contacted an energized line and was electrocuted. Employee #1 had failed to de-energize and ground the 277-volt electrical circuit on which he was working.
