Investigation Summary
On May 12, 2005, Employee #1 was removing a blockage from a cement plant using Cardox tubes. After clearing the blockage and removing the Cardox tube, hot material spewed onto Employee #,1 resulting in serious burn injuries to his arms, neck, and ears. Employee #1 was hospitalized. He was not wearing any protective equipment.
