Investigation Summary
Employee #1 was a member of a crew laying block at a new residential construction site. Employee #1 and two coworkers were working from a 12-ft high fabricated frame scaffold that was placed against the block wall. Another fabricated frame scaffold, that was approximately 10 ft in height, was placed at an angle from this scaffold against a wall that was perpendicular to the first wall. Both scaffold systems had a number of deficiencies, such as no fall protection was provided, and the planks were damaged and incorrectly supported and placed. They were not placed on firm foundations, and were not plumb, level or square. There was no safe means of access between the two scaffold systems. When the crew completed their work on the 12-ft high scaffold, they moved to the lower scaffold. They did this by climbing down the end frame of the taller scaffold onto the end frame of the shorter scaffold, and then stepping onto the planks of the shorter scaffold. The two coworkers successfully accessed the shorter scaffold, but Employee #1 fell during his attempt. Employee #1 landed face first onto the ground and was pronounced dead at the scene from his injuries.
