Investigation Summary
Employee #1 was helping a coworker on a pneumatic head press when his left hand was caught in the second tank clamping device. Employee #1 suffered the amputation of his little finger and crush injuries to part of his hand. The operator had two hand controls for the closing cycle and the operation was supposed to be a one person operation, but when the shifts overlap, operators regularly help out banging the head of the tanks in after the tank body is clamped in the head press. Employee #1 was hospitalized and the finger was able to be reattached.
