Employee Amputates Fingers on Unguarded Chain Mechanism

At approximately 2:00 a.m., Employee #1 was working alone in the No Bake Molding Area. Employee #1 had been employed at Richland Center Foundry as a Rollover Operator less than two months. At the time of the accident, Employee #1 was stepping up onto a motor-driven conveyor to remove two plugs from the top of a mold. Standard practice involved disengaging the power to the conveyor prior to stepping onto the conveyor, although no written procedures for this task were developed. At the time of the accident, Employee #1 had not disengaged the power. Upon stepping onto the conveyor, Employee #1 slipped and fell forward, catching their right hand in the unguarded chain mechanism that powered the conveyor rollers, amputating all four fingers on their right hand.
