Employee Is Crushed and Killed in Guide Rollers of Machine

At approximately 6:00 a.m. on November 11, 2005, Employee #1 and a coworker were applying grease to several pieces of equipment in the shredding area of a facility. Employee #1 was a temporary labor worker and the coworker was an employee of the company. According to the coworker, he and Employee #1 were going through the building and greasing machinery that could be reached from ground level without removing guards or safety devices. There was additional equipment the needed lubrication but guards had to be removed, and a lockout procedure was normally utilized for this practice. After separating to do their work, the coworker noticed that one of the machines was not running. He investigated and found Employee #1 caught in the guide rollers of the trammel with only his legs visible. Employee #1 died from massive crushing injuries to his upper body. The trommel was guarded, but the guard was not affixed or attached at any point. In addition, a lockout procedure had not been used at the time of the accident.
