Investigation Summary

Investigation Nr: 200690774
Event: 11/15/2005
Employee Is Killed When Caught in Auger

On November 15, 2005, a crew of three employees was preparing a site for a single family home. They were using an EB-60 blower truck to fill 800 to 1,000 ft of "filter sock" with wood mulch. A filter sock is a 12-in.-diameter, biodegradable tube, filled with mulch, and used for erosion control. Inside the blower truck's body was a conveyor that moved the mulch to the rear of the truck, where two augers and agitators fed the mulch to a blower hose. Employee #1 was killed when he climbed into the back of the truck, apparently to free a jam, and was pulled into the augers.
