Investigation Summary
At approximately 2:00 p.m. on January 3, 2005, an employee crawled beneath a 12-ft wide by 55-ft. long mobile office trailer to weld the metal tongue back onto the metal frame of the trailer. The trailer was being moved to another location so all underpinning had been removed, and the trailer was supported by four hydraulic tracks at the middle and at the end and with w piers, consisting of two 12-in. hollow cinder blocks and one 4-in. solid block, at the other end. The employee was under the trailer preparing to weld the tongue when the weight of the trailer crushed the solid block allowing the beam it was supporting to fall 3.5 in. to the 12-in. hollow cinder block which was crushed under the sudden weight. When the pier failed, the 3-in. wide by 10-in. high support beam crushed the employee resulting in his death approximately one hour later.
