Employee Sustains Amputation of Foot Unloading Forklift

Employee #1 climbed on top of the forklift cab putting him on the backside of the pallet to push off the boxes. When the mast was lowered Employee #1's foot was pinched between the mast and the base of the lift. Employee #1 sustained an amputation and was hospitalized.
