Employee Dies in Trench Collapse

Employee #1 went into a 10-ft deep by 24-in. wide footing to remove excess dirt left by a track hoe. The side of the footing collapsed and buried him under approximately 4 feet of dirt. Employee #1 died of asphyxiation.
