Employee Is Killed When Struck by Truck

At approximately 10:52 a.m. on November 16, 2004, Employee #1 was conducting flagging operations and directing work along a low volume rural road. After directing a driver of a tandem dump truck to proceed backwards to the east, Employee #1 was walking parallel with the truck. Employee #1 was speaking on the phone while walking to the east. When Employee # 1 started to walk in a northeast direction, he was struck by a portion of the axle of the front wheel of a truck. Employee #1 sustained head injuries, which resulted in his death.
