Employee Is Killed When Struck by Hoe Ram

On December 14, 2004, Employee #1, a public utility worker, was leaning over a 12-in. iron water pipe, wrapping a wire rope around it for a track hoe to lift. When the track hoe operator leaned forward, his coat accidently caught the right lever, which immediately lowered the boom with the attached hoe ram. The hoe ram struck Employee #1, crushing him between the iron pipe and the hoe ram. Employee #1 was killed.
