Employee Falls Through Floor Opening Dies

On October 21, 2004, Employee #1, a county building inspector was conducting a rough-in inspection of the framing, plumbing, and mechanical systems at a residential house under construction. He was on the second floor walking backwards looking up at an HVAC ducts and wiring while taking notes. As he was doing this, he stepped through a 5.5-ft by 6.75-ft opening in the floor and fell 10.58 ft backwards head first onto the concrete floor below. He was hospitalized and died later.
