Employee Is Killed When Pinned by Forklift

Employee #1 was using a "finger lift" (forklift) to unload pasteboard boxes from a truck. When Employee #1 finished unloading the truck he went from one area of the warehouse to another area used for storage of the finger lift (forklift). Employee #1 drove along a passageway 80 ft long and 5 ft wide. Employee #1 was riding approximately 32 in. above the parking floor. Something happened to Employee #1 that caused him to fall to the parking floor. Employee #1 was pinned by the forklift and was killed.
