Investigation Summary
At approximately 11:00 a.m. on October 6, 2005, Employee #1 was working alone, running Machine Number 18 Johnson, a part revolution mechanical power press, to make metal parts. The press did not have the control reliability feature that prevents the machine from cycling in the event of failure on one of the parts. Employee #1 had to insert the part and take it out after a cycle. The chain that drives the crankshaft of the machine came off from the sprocket, disabling the brake monitoring system, and allowing the press to run continuously. The press cycled several times and caught Employee #1's fingers at the point of operation, amputating them. He was hospitalized for his injuries.
