Investigation Summary
At approximately 12:15 p.m. on April 23, 2005, Employee #1 was standing next to an injection molder. One end of the molder was on the forks of a forklift and the other end was on rollers that were typically used for moving machines. As a coworker was operating the forklift, the injection machine tipped off the forks of the forklift, trapped the employee's foot under it, struck him in the chest and killed him. Although this was a commonly performed task, the company did not have written procedures on machinery moving.
