Investigation Summary
On February 3, 2005, a crew was assembling a front wall, approximately 8 ft by 14 ft, laid on the first floor deck. Employee #1 was walking backward, carrying a 2 ft by 8 ft plywood. He hit the plate, which was at the end of the wall and tripped. Employee #1 fell with head on into a 42 in. by 60 in. opening, approximately 9 ft above the basement. He sustained concussion to his head and was air lifted to Lutheran General Hospital, where he was hospitalized.
