Employee fractures finger in spinning fixture

At 7:45 a.m. on October 7, 2004, Employee #1, a spinner machine operator, was operating a spinner machine that was designed with a square cutout to house a rectangular cardboard box. While the machine was spinning, the operator loaded insulated wire into the box. The wire was placed in a circular position. Employee #1's little finger was caught between the outside of the box and the rectangular cutout of the machine and was fractured. Employee #1 was hospitalized. The gap between the outside edges of the box and the cutout that it was set in was too large, thereby catching the employee's finger. The company had a safety program and provided training to their employees.
