Employee Is Injured When Struck by Forklift

At approximately 3:00 p.m. on October 14, 2004, Employee #1 was standing in the vicinity of a forklift retrieving oil from a barrel for machining operations. A worker was operating a forkliftto move three bins of screws that were stacked. The forklift was a Nissan, Model Number KCPH02A25PV, Serial Number KCPH02P902861. The bins had a combined weight of about 6,800 lbs, which exceeded the forklifts capacity of 4,400 lbs. As the worker moved the bins, they fell off the forklift and struck Employee #1. Employee #1 was hospitalized and treated for a broken leg. The accident investigation revealed that the employer did not have a forklift training program. Additionally, the root cause of the accident was from lifting a load that was over the forklift's capacity.
