Employee is killed when he falls through a skylight

On September 17, 2004, Employee #1 and a coworker were moving an outrigger scaffold on the inside of a building. Employee #1 was walking over a glass skylight with a 50-pound scaffold counterweight when the skylight broke. Employee #1 fell 75 ft and died.
