Employee's Strikes Head Against a Molding Machine and Dies

On October 12, 2004, Employee #1 was making an adjustment to an extrusion molding machine to adjust the flashing when he slipped. His head was caught between the hydraulically operated side of the mold housing and the fixed elevated work platform. A coworker removed Employee #1 when the next automated cycle began. Employee #1 was conscious when removed from the machine, but collapsed and died a few minutes later.
