Investigation Summary
At approximately 1:10 p.m. on July 29, 2004, an employee was pressure testing a roll force cylinder. During the test, he was struck in the chest and killed by either high-pressure hydraulic fluid or the end of the hydraulic hose. The cylinder was pressure tested with up to 4,500 lbs per square inch of hydraulic fluid.
