Employee is injured in fall from scaffold

On August 24, 2004, Employee #1, a scaffold erector, was overturning scaffold platform planks, one-by-one, to remove concrete debris and spillage. After he reset each plank, he would move to the next one and overturn it. When he stepped on a plank that he had failed to secure properly, he fell through the scaffold, 14 ft to the ground. Employee #1 was hospitalized for a fracture.
