Forklift Falls Off Dock and Severely Injures Operator

On August 25, 2004, an employee operated a forklift on a loading dock. He loaded a truck that was not chocked and which was not equipped with either air brakes or a functioning hand brake. The truck shifted forward, permitting the forklift to fall into the gap between the truck's tailgate and the dock. The forklift operator was ejected from the forklift. He was hospitalized with fractures to his face and skull, he sustained a concussion, and cervical spinal injuries.
