Employee Killed When Pinned Between Tailgate and Truck Bed

When the normal day shift ended at 4:00 p.m., several attempts were made to contact Employee #1 on the 2-way radio with no results. At approximately 4:45 p.m., the Supervisor asked a coworker to go to the waste water plant to contact Employee #1. At approximately 5:00 p.m., the coworker arrived at the location the sweeper truck is normally dumped and observed the Sweeper with the bed raised into the dump position with the front (drive) engine idling and the rear engine, which provides power for the vacuum, hydraulic and water systems operating at approximately 2,000 RPMs. The coworker observed Employee #1 pinned between the bed and tailgate of the Street Sweeper. The coworker attempted to contact Employee #1 by calling his name and received no response. The coworker returned to his vehicle and radioed for help on his 2-way radio and drove 0.45-miles to the waste water plant and enlisted the assistance of another coworker. They proceeded back to the scene where the coworker directed the other worker to hold Employee #1 while the coworker operated the hydraulic control lever to open the tailgate, releasing Employee #1. The second coworker removed Employee #1 from the tailgate and placed him on the ground. The first coworker then moved the Sweeper forward 18.5 ft to clear the area. The coworkers checked for a pulse with negative results and found Employee #1 to be cold and stiff. Rescue personnel responded at 5:06 p.m. and determined that Employee #1 was deceased.
